Autumns View Personal Care, Portage’s newest and spacious facility, offers quality care through highly qualified and caring staff including Licensed Practical Nurses and Certified Nursing Assistants. The staff at Autumns View will assist residents to obtain and maintain their highest level of physical and mental functioning. !
Located in Maple Winds Care Center, Autumns View offers many services to meet the needs of our residents.
• Transportation services are provided for residents to assist with their scheduled medical appointments such as podiatry, audiology, dental and optometry services.
• Laboratory and x-ray services are available on site.
• Full service beauty/ barber shop located at the facility for each resident’s convenience.
• Telephone and cable hook-ups are available in each room to provide privacy and comforts for all residents.
Residents will enjoy socializing with friends and family in the dining area for delicious, home cooked meals that meet their nutritional needs.
Residents will also enjoy activity programs geared toward their individual likes as well as adapting to their individual levels of functioning.
A full range of therapy services are available on site for those residents that may need out-patient rehabilitation to remain as independent as possible. Therapy services can include physical, occupational, and speech therapy.
If a resident would require skilled nursing care, they would move smoothly into Maple Winds Care Center through our continuum of care process.
Due to Autumns View being located at Maple Winds, it is well equipped with a state- of -the –art fire protection system and a generator for emergency power to ensure the safety of all of our residents.
