Mulberry Square is located in downtown Punxsutawney, Pennsylvania, and is close to the hospital, emergency medical services, home health care, and physician offices. The building itself is a one-story brick structure with a large central livingroom, elegant diningroom, large activities room which opens into a beautiful protected courtyard. The courtyard has a gold fish pond, stunning flower gardens, and elevated vegetable gardens, which can be attended to by the residents if they wish.

Mulberry Square is a 75-bed skilled nursing facility along with an 8-bed assisted living unit. This provides our residents with peace of mind, knowing that they can transfer from assisted living (personal care) to skilled care when appropriate and vice versa. Large semi-private and private rooms are available and can be furnished with the residents' personal items if desired.

Mulberry Square provides short-term rehabilitation services for those individuals who need additional rehab after discharge from the hospital. You can learn more about our rehabilitation service by clicking onto rehab services from the main menu or the quick facts menu.

Respite care is available and is 24-hour care in a skilled nursing and rehabilitation center. It is designed to provide families with a helpful break from the responsibilities of caregiving. Respite Care can be used for a variety of reasons - for a weekend away, a family emergency, a long-awaited vacation, or business travel that interferes with caregiving. Many caregiving families also use Respite Care as a way to "try out" full-time nursing center care without making a commitment to a long-term placement. You can be confident that your loved one(s) is well cared for in your absence.

REHAB UNIT

Our premier therapy team, Guardian Rehabilitation Services, provides a catalyst for wellness for those who need services for intensive rehabilitation following a stroke, heart attack, fracture or other disability. Specializing in short term post hospital care, our licensed Physical Therapist, Occupational Therapist and Speech Therapist work as a team to ensure each individual receives the maximum benefit of rehabilitative treatment from the day of admission to the day of discharge.

Services provided include:

Physical Therapy focuses on body movement dysfunctions. Physical Therapists assist residents with walking and developing the physical ability for bed and wheel chair transfer techniques. We have a fully equipped therapy room with specialized equipment to help residents become confident in their mobility skills.

Occupational Therapy teaches the resident self care and promotion of activities of daily living skills such as dressing, bathing, food preparation and eating. Therapists help residents develop maximum use of arms, hands and fingers by addressing both thought (cognitive) and visual (perceptual) deficits that interfere with everyday skills.

Speech/Language Pathology focuses on the improvement of a resident's communication skills. Our Speech Pathologist provides basic retraining of cognitive skills and independence through decision-making, reasoning and memory. Oral motor strengthening, swallowing retraining and written expression are also provided to maximize an individual's potential following an injury or illness.

Respiratory Therapy helps residents who have an increased risk to respiratory difficulties. Our specially trained nursing staff works with the residents to strengthen muscles, clear congestion, aid in ventilation and help in the prevention of further difficulties.

For residents who no longer require skilled therapies, our Restorative Nursing Program can maximize any resident's potential for both physical and mental well being. Such programs as Range of Motions, Walk to Dine, Therapeutic and Cognitive Activities, Hair Care and more, provide the reassurance to each resident that feeling good is not just for the physical body but the mental, emotional and social bodies as well.


ACTIVITIES

Our Activities Department is made up of highly qualified and specially trained personnel. It is the goal of our Activities Department to provide entertaining and stimulating therapeutic activities that meet the unique physical, social, emotional and spiritual needs of each resident.

Recreational activity is more than just arts and crafts. It is a skilled discipline that helps residents recover memory and physical functions through specially designed activities and recreational programs.

Residents can participate in a variety of group or individual activities. Activities, such as art, gardening, music therapy, and pet therapy, are designed to meet residents' interests and needs. Our activities personnel are trained to understand the special limitations of the elderly, and to encourage all residents to participate at some level.

Individuals who are withdrawn, disoriented or anxious can be helped by remotivation therapy, which uses visual, auditory and olfactory cues to trigger emotions or memories. Reality orientation helps residents confirm basic facts, such as the current date and place. By establishing simple, positive goals, we can help many residents become more independent.
