FACILITIES/SERVICES

Basic Fees

- Tri/Quad - Daily Fee: $65, Monthly Fee: $1,950
- Semi-Private - Daily Fee: $80, Monthly Fee: $2,400
- Private - Daily Fee: $125, Monthly Fee: $3,750

**when the resident’s level of care increases, Heartland will increase his/her monthly charge accordingly as explained on the levels of service and care information below.

Additional Charges

* Care Level I - $0
* Care Level II - 20% of base rate
* Care Level III - 30% of base rate

- damage to other’s belongings
- annual flu vaccination
- beautician and barber services
- respiratory therapy and oxygen therapy
- repair services for personal belongings
- dietary supplements
- incontinence care items
- physician, podiatry, dental, and eye care fees
- telephone (hookup and monthly expenses)
- cable television in rooms
- hygiene and grooming items
- skilled therapies
- prescriptions and otc drugs
- audiology services
- dry cleaning
- wound dressing
- laboratory and xray fees
- trips/transportation (not already planned activities)


Levels of Service and Care

Level 1 - Base rate as indicated above

The resident meets the following standards:
* Continent
* Feeds self
* Needs assistance with medications
* Self-ambulating with minimal assistance

Level 2 - Additional charge of 20% of base rate per day.

The resident meets the following standards:
* Use of walker
* Oxygen therapy
* Occasional incontinence / catheter
* Needs some assistance eating meals

Level 3 - Additional charge of 30% of base rate per day.

The resident meets the following standards:
* Incontinence
* Needs complete assistance eating meals
* Wheelchair and assistance with ambulation
* Complete hygiene care
* Special treatment (i.e. Ostomy care, etc.)
