ASSISTED LIVING
Maintain Your Independence With the Added Support From Our Staff.
Personal Care living at SMH was developed to provide assistance with activities of daily living to residents who do not require the full medical services of a nursing facility. Our services require a delicate balance between reducing risk, which may accompany the aging process, while respecting and promoting our residents' desire to remain independent.
Feeling at home at SMH starts with having the comforts of home in mind in our semi-private and private rooms. Our private rooms are near social living rooms and country-style kitchens, promoting a strong sense of community.
Our Personal Care facility offers comprehensive amenities, services and assistance.
Assistance with daily living activities is provided by trained nursing assistants available 24 hours a day for the monitoring of medication, proper nutrition, etc.
Residents can choose from rooms that accommodate singles or couples, and can choose from a wealth of community areas to enjoy.
Located right on site, SMH offers full-time social services support with licensed physical, occupational and speech therapists.
Park View ... Preserving the Character of Each of Our Residents
Since 1917, SMH has made a commitment to provide care to community residents. We are extending that commitment by providing an innovative program that is designed to meet the unique needs of persons with dementia.
Park View, the Special Care Dementia Unit at SMH, provides a tailored environment created specifically to promote self-worth, dignity and wellness of persons with dementia by concentrating on each individual's abilities.
SMH's philosophy of care at Park View is one of flexibility, preserving each individual's identity, and understanding and accepting that dementia is a disease, not a state of mind.
To meet the unique needs of persons with dementia, SMH is using the psychosocial model of Personal Care by providing a specially designed environment, a flexible program of care and activities, and a staff educated in dementia care. These three elements, in addition to the involvement of family and friends, combine to create an unobtrusive wellness program that promotes independence and provides avenues for our residents to enjoy the sense of community contribution.
NURSING
While no facility can take the place of the home, which you have made and shared so many memories, we invite you to begin to establish new friendships and create new memories with SMH.
Our Nursing facility is designed to meet all residents' physical and medical needs, offering three meals per day as well as diverse activities for all to enjoy.
Our staff of licensed nurses is on duty around the clock monitoring all medication and nutritional needs, while trained nursing assitants provide support with activities of daily living.
