The Patriot is a progressive, nonprofit Nursing and Rehabilitation Facility operating 100 fully licensed nursing beds and 76 Personal Care/Assisted Living accommodations. All 100 nursing beds are Medicare and Medicaid certified. We accept a variety of insurances; we also accept private pay status. To complement our adult services, an in-house child Daycare with "The Patriot Kids" offers service to employees and the general public.
Various levels of healthcare, individualized to need, are provided to age ranges from the young adult through the senior citizen. An independent practitioner serves as Medical Director, lending her expertise to our mission.
Our aggressive, fully staffed Rehabilitation Program operates six days per week. Therapy services are offered not only to those recuperating from procedures such as hip and knee surgery, but also to those needing to regain strength and endurance from diagnosis such as pneumonia, heart attack and stroke.
A Palliative Care Program utilizes licensed Hospice Services as needed; our Serenity Suite offers emotional and physical support to the resident and family during the last days of life. Monitoring and intervention by licensed staff and support personnel is provided around the clock.
Intergenerational programming, the connection between residents and The Patriot Kids, is an eagerly anticipated event occurring throughout the week.
Our Personal Care level offers a supervised, homelike setting that can accommodate the highly independent individual through those requiring an increased level of assistance. Two Independent Living Suites, complete with a kitchenette and private bath, have recently been completed.
Respite Care is available within the Nursing or Personal Care component. This allows the much needed break for a caregiver, with their loved one being cared for in an environment that is responsible, comfortable and conveniently located.
PERSONAL CARE
As part of The Patriot, our Personal Care/Assisted Living Facility is located in the heart of Somerset; convenient to multiple shopping areas. We are only minutes from Somerset Hospital.
Two recently constructed Independent Living Suites, complete with kitchenettes and private baths, are in addition to private and semi-private rooms. Accommodations are comfortable, spacious and newly carpeted. Rooms are fully air-conditioned with individual controls. Two elevators are available for resident use. The floor contains a full sprinkler and alarm system.
A daily breakfast buffet is served to the residents. There are in-house cable and telephone systems; Internet access is available in the large Atrium. Laundry service can be requested. Designated resident parking assures a close access to the building.â€
Personal Care Level staff is comprised of LPNs, trained Medication and Personal Care Aides. Rehabilitation Therapies (Physical, Occupational and Speech) are on-site. Select physicians do offer service at the facility. Pharmacy services that deliver are available. Designated Activity staff serve this level of care. Short Term/Respite Care can be arranged.
Two additional levels of care within the facility ease the transition, should a higher level of care become necessary for the resident on either a temporary or permanent basis.
A variety of payment options is available. We will assist with application for Veteran's Administration benefits, as applicable.
SUPPORT SERVICES
It takes a village to do it right. Resident care takes a variety and number of personnel. Support services of Dietary, Laundry,
Housekeeping and Maintenance quietly operate behind the scenes to make sure that the needs of our residents are met and that a safe, secure and clean environment is provided within the building.
ACTIVITIES
The Patriot boasts a dedicated Activities Department, complete with a Director and “department-specific†staff. Individual and group activities are available, targeting the resident’s interest and ability. The smiling faces of volunteers are always welcomed!
The Barber and Beauty Salon keeps our residents looking their best, and offers that little bit of extra pampering.
Personalized shopping services are available.
Telephone and cable access is available in all rooms.
A licensed child Daycare Center is on-site. Intergenerational programming benefits both the residents and “The Patriot Kids.†Learning, laughter and fun knows no age.
